カルタヘナ議定書第一回締結国会議において、様式１２（遺伝子組換え生物等の使用等の規制による生物の多様性の確保に関する法律施行規則に定める様式１２）について、記載すべき内容の詳細が追加的に定められました。以下の説明をよく読んで記入下さい。

（1）施行規則様式第12の第1欄には、”Living modified organisms の下に Destined for contained use" と記入した上で、遺伝子組換え生物等の名称を括弧書きで記入すること。また、国際的な識別記号（注）が付されているものの場合にあっては、その記号を括弧内に記入すること。

（注）国際的な識別記号とは、議定書のホームページ等において特定の遺伝子組換え生物等に関する情報を容易に検索できるようにするために用いらるものであり、現在は、経済協力開発機構〈OECD）において開発された商業化段階にある遺伝子組換え植物に適用されるものが存在している。

（2）同第2欄には、輸出しようとしている遺伝子組換え生物等が危険物輸送に関する国連勧告、国際植物防疫条約又は国際獣疫事務局における国際家畜衛生規約において措置が求められているものの場合には、これらの勧告等における区分又は措置の内容を記入すること。特にこれらの措置が求められていない場合には、その旨記入すること。

（3）同第3欄の（2）の輸入者の項には、輸入者が仕向先と異なる場合には、その仕向先である個人又は団体の氏名又は名称、住所又は所在地及び連絡先についての詳細を併せて記入すること。

(a） LMOs for contained use（paragraph2（b），Article 18）：
（i） Clear identification as "living modified organisms” including common and scientific name of the organisms and as "destined for containeduse";
(ii) The name and address of the consigee，and exporter or importer，as approciate，including contact details necessary to reach them as fast as possible in case of emergency；
(iii) Any requirements for the safe handling，storage，transport and use of the LMOs under applicable existing international instruments，such as the United Nations Recommendation on the Transpor to Dangerous Goods，IPPC and OIE，domestic regulatory frameworks or under any agreements entered into by the importer and exporter．In the event that there is no requirement，indicate that there is no specific requirement；
(iv) Where appropriate，further information should include the commercial names of the LMOs if available, new or modified traits and characteristics such as event(s) of transformation，risk class，specification of use，as well as any unique identification where available，as a key to accessing infomation in the Biosafety Clearing House．
